On the Job Injury Program

One of the most important areas of Risk Management is the safety and the well being of employees. When employees are injured in the course and scope of their job, they are provided for through the City's On-The-Job Injury benefit program. Our goal is to help employees work in a safe, productive, friendly environment.


EMPLOYEE INJURY REPORT(EIR) FORMS & ACCIDENT/INCIDENT REPORT (AIR) FORMS MUST BE REPORTED TO YOUR SUPERVISOR & COMPLETED ON EACH INJURY AND/OR ACCIDENT WITHIN YOUR SHIFT OR 24 HOURS OF THE INJURY.

Employee Assistance Program (EAP)

The EAP allows five free visits to a counselor through Pathways Behavioral Health Services Employee Assistance Program (EAP). Services are private and confidential. They have eight offices available.
